To register, either print and fax or save and email the completed form. Please use the address and fax number listed on the form.
Registration:
Conference registration includes admission to technical sessions,
continental breakfasts, coffee breaks, lunches, and all conference
material. Additional fees are required for the optional training courses
Payment:
Payment methods accepted are Visa, MasterCard, personal and company
cheques. Payment must be in Canadian funds and cheques must be made
payable to SCAV-CSVA.
Confirmation:
Conference registrations made online will receive an e-mail confirmation
of registration. Individuals who mail their conference registrations and
payment via postal delivery will receive written confirmation of their
registration. The confirmation of registration will serve as your
receipt for payment.
Cancellation policy:
Notification of cancellation must be submitted in writing, e-mail or fax
to the CSVA office no later than November 2, 2009. Cancellations by this
date will be subject to a cancellation charge of $25. No refunds will be
given for any cancellations received after November 2nd.
